SERVICES DESCRIPTION REQUIREMENTS
Absolute Assignment

By filling a written notice satisfactory to the Company, the Policy Owner may assign this Policy. The Company shall not be considered to have knowledge of an assignment unless the original or duplicate thereof is filed with the Company. Once the policy monies, wholly or partly, have been pledged as security or assigned to a person, the claim of the person entitled under the security or assignee shall have priority over the claim of the nominee subject to the rights under the security or the assignment being preserved. The appointed assignee must be an individual (adult) and authorised financial institution / non-financial institution.

Permitted relationship for Absolute Assignment

Individual:

  • Spouse
  • Children
  • Parents
  • Legal Guardian
  • Grandparents
  • Grandchildren
  • Siblings
  • Siblings of parents

Corporate:

  • Employer/Employee
  • Bank/Regulated financial institution in Malaysia (as collateral)
  • Public and private trustee in Malaysia
  • Absolute Assignment Form
    (Effective 1st September 2023, only stamped Absolute Assignment forms will be accepted for processing.)

Individual

  • Copy of identity card / current passport Assignee's (Individual)
  • Proof of relationship (Birth certificate, Marriage certificate)

Corporate

  • Certificate of Incorporation (Public/Private company)
  • Return of allotment of share
  • Return giving particulars in register of directors, managers, secretaries, and changes of particular
  • Authorization letter from the company or resolution from board of directors for the appointed authorized person
  • Copy of identity card/passport of authorized person

  • Proof of relationship (Employment letter, Loan offer letter, Appointment letter)

Change in Payment Mode

Change in payment mode is allowed at the next premium due date. For Standing Instruction, Policy Owner need to advise the bank on any changes in contribution.

Change in Fund / Premium Re-direction

The choices of fund into which the premium is directed can be changed at any time. Processing fee is charged RM 50.00 per transaction by cancelling units (subject to plan). Change will take effect on the next regular premium due date.

Change in Regular Premium / Layer Premium

Change in regular premium is allowed at any time. The change will take effect at next premium due date. The minimum regular premium/layer premium amount is RM 50.00

Change in Regular Top-up (MaxSaver)

Change in regular top-up is allowed at any time. The change will take effect at next premium due date. The minimum regular top-up amount is RM 10.00

Change of Contact Details

The Policy Owner may request to change the following details:

  • Mobile / Residence / Office No.
  • E-mail Address
  • Correspondence address
Change of Signature

In the event of Policy Owner want to replace the signature signed previously to a new signature (either during application or policy inforce), he / she may request to change of signature.

Change of Policy Details (Without Premium Adjustment)

The Policy Owner may request to amend the following details:

  • Name
  • NRIC No.
  • Nationality
  • Race
Decrease in Basic and Rider Sum Assured

Policy Owner may opt to reduce his / her basic or rider Sum Assured at any time. The change will take effect on Next Premium/Insurance Charge due date.

Deletion in Rider(s)

Optional Rider(s) attached to the basic Policy may be deleted at the Next Premium/ Insurance Charge due date.

Addition of Rider(s)

Optional Rider(s) may be attached to the basic Policy which is currently in force, subject to its eligibility and Company’s rules. The change will take effect on Next Premium / Insurance Charge due date.

*Addition of rider(s) is subject to Underwriting assessment.

Increase in Basic and Rider Sum Assured

Policy Owner may opt to increase his / her basic or rider Sum Assured at any time. The change will take effect on next Premium / Insurance Charge next due date.

*Increase in Sum Assured is subject to Underwriting assessment.

Irregular Top-Up (Additional Premium)

The Policy Owner may one-off top up at Policy inception and any time after Policy is purchased. Any additional premium shall be allocated to the Zurich Life Insurance Funds as stated in the Top-Up Application Form. Top up Premium is subject to Service Fee of 5% and Additional Fee of RM 25.00 per top up transaction applicable to selective plan.

Appointment of Nominee & Trustee

The Policy Owner may nominate his/ her family member(s) or friend(s) to receive his / her policy proceeds.

Partial Withdrawal

Policy Owner is given the flexibility to make partial withdrawals from a portion of the accumulated Investment Account value or accumulated declared dividend prior to policy maturity.

i) Investment Linked Plan

This benefit is subject to a minimum partial withdrawal amount and maintained a minimum investment balance in the Investment Account. Partial withdrawal fee is chargeable and shall be deducted from the withdrawal amount.

Please note that the minimum partial withdrawal amount, minimum investment balance and withdrawal fees may vary depending on the plan.

ii) Ordinary Life Plan

This benefit is subject to a minimum partial withdrawal amount of RM500 or full withdrawal of the declared dividend amount.

Reinstatement

If a premium in default beyond the grace period and the Policy has not attained cash value, the Policy is said to have lapsed.

Policy may be reinstated within six (6) months up to five (5) years from the Lapse date depending on the plan.

Note: Subject to Underwriting assessment.

SPM Allowance

For Educational plan that eligible for SPM allowance, Policy Owner to claims the SPM allowance based on the Insured’s SPM examination results (Or its equivalent). The Policy effective date should be prior to the exam date and the policy should be inforced until current date.

3 to 5As – RM 1,000.00

6As and above – RM 2,000.00

The benefits is payable on per life basis.

  • Letter signed by Policy Owner
  • Certified True Copy of SPM exam result’s slip
  • Copy of Identity Card / Current passport of Policy Onwer  
  • Complete Electronic Fund transfer form or update Bank details via MyZurichLife
Self-Certification Form of Tax Residency (Individual and Entity) Individual or Entities who declared Tax status as Foreign Tax Resident. 
Foreign Account Tax Compliance Act (FATCA) Declaration Individual or Entities who declared as resident or tax payor of USA.
Switching of Units (Transfer of Units)

Policy Owner may instruct the company to switch all or some of the allocated Units of the Investment Account in a particular Zurich Life Insurance Fund to another Zurich Life Insurance Fund. The election of such switch is subject to fees, charges, and conditions that the Company may impose from time to time. One free switch is available every year. Subsequent switches incur a processing fee of RM 50.00 per switch by cancelling units. Switching fee is waived if policy owner performs switching via MyZurichLife.

Update of E-Invoicing Details

The Policy Owner may request to update the e-invoice related data :

  • Company registration number (BRN) - new 12-digit format
  • Malaysia Tax Identification Number (TIN)
  • Sales or Services Tax (SST)
  • Registration Number
  • MSIC code
  • Tourism Tax Registration (TTR)

 

Declaration of Authorized Person and Beneficial Owner Declaration Form
Business entities are required to declare the Beneficial Owner, who is a natural person, and the Authorized Person, who is the person that will representing the entity under for the policy.
  • Declaration of Authorized Person and Beneficial Owner Declaration Form

    To attached together with below supporting document :
  • Certified Copies of Company Registration (e.g., Form 9)
  • Certified Copies of Identification (e.g., NRIC or passport) for the Authorized Person and Beneficial Owner
  • Official Company Authorization Letter regarding the appointment of the Authorized Person or the Board Resolution related to this appointment.
  • Certified Copies of documents that demonstrate the Latest Shareholding or Directorship details (e.g., Form24/49, Trust Deed, Minutes of Meeting, etc.)

Request for Policy Surrender Form

Please be reminded that you would lose the benefits under your policy in the event of surrender/termination of a policy, we encourage you to reconsider the risk prior to continuing with surrender. Please visit any of our branches to make an informed decision as we may have an alternate solution for you, or contact our Call Centre at 1-300-888-622 or email to callcentre@zurich.com.my for further advice and assistance.

Change in contribution payment mode is allowed at the next contribution due date. For Standing Instruction, Participant should need to advise the bank on any changes in contribution.

Change in Fund / Contribution Re-direction

The choices of fund into which the contribution are directed can be changed at any time. Processing fee is charged RM 50.00 per transaction by cancelling units (subject to plan). Change will take effect on the next regular contribution due date.

Change of Signature

In the event of participant unable to sign or remember the signature signed in application form, he / she may request to change of signature.

  • Request For Change Form (Without Contribution Adjustment)
  • Note: the request to change of signature is required to be declared at any nearest Zurich Takaful branches and the form has to be witnessed by one of the following personnel for process identification.
    • Marketing Regional Head
    • Marketing Head
    • Branch Personnel
    • Servicing Wealth Planner
    • Agency Manager (Immediate leader of Servicing Wealth Planner)
    • Chief Agency Manager (Highest leader of Servicing Wealth Planner)

Change of Certificate Details (Without Contribution Adjustment)

 

 

 

The Certificate Owner may request to amend the following details:

  • Name
  • NRIC No.
  • Nationality
  • Race

Participant may opt to reduce his / her basic or rider Sum Covered at any time. The change will take effect on next takaful charge next due date.

Deletion in Rider(s)

Optional Rider(s) attached to the basic Certificate may be deleted at the Next Contribution / Takaful Charge due date. However, package riders are not allowed to be deleted on request basis.

Duplicate Certificate (loss of original Certificate)

When a Certificate Contract is lost beyond hope to recovery, or is wholly destroyed, the Participant can apply for a duplicate Certificate.

Addition of Rider(s)

Optional Rider(s) may be attached to the basic Certificate which is currently in force, subject to its eligibility and Company’s rules.

*Addition of rider(s) is subject to Underwriting assessment.

Increase in Basic and Rider Sum Covered

Participant may opt to increase his / her basic or rider Sum Covered at any time. The change will take effect on next takaful charge next due date.

*Addition of reder(s) is subject to Underwriting assessment.

The participant may one-off-top up at certificate inception and any time after Certificate is purchased. Any additional contribution shall be allocated to the Zurich Takaful Funds as stated in the Top-Up Application Form. Top up Contribution is subject to Service Fee of 5% and Additional Fee of RM 25.00 per top up transaction applicable to selective plan.

Nomination (Hibah)

Gift of certificate moneys to loved ones made during the Participant’s / Certificate Owner’s lifetime. Giving one’s wealth to others without the expectation of any replacement or exchange with the transferring effect on the ownership during lifetime or executed upon death. It is applicable either to family members (legal heirs) or members who are not entitled to inheritance. Hibah is applicable to adult and juvenile. Hibah is also applicable for both Muslim and Non-Muslim. As for juvenile, Participant must appoint the trustee (adult).

Nomination (Wasi)

A nomination made by Participant to his / her nominee with the intention to appoint him/her as the Executor. The person(s) named can only act as an administrator / executor (Wasi) who will be responsible for distributing the person covered’s monies to the rightful heirs in accordance to the Faraid Law (Distribution Law for Muslim) Page 6 of 8 or Distribution Act 1958; as amended by the Distribution (Amendment) Act 1997 for Non-Muslim. Wasi is applicable to adult or authorised public trustee.

Partial Withdrawal

Participant is given the flexibility to make partial withdrawals from a portion of the accumulated Participant Investment Account (PIA) value prior to certificate maturity.

This benefit is subject to a minimum partial withdrawal amount of RM 500.00, and a minimum investment balance of RM 1,000.00 must be maintained in the PIA. Please note that the minimum partial withdrawal amount and minimum investment balance may vary depending on the plan.

There is a Partial Withdrawal Fee of RM 50.00 per withdrawal and the fee shall be deducted from the withdrawal amount.

 
Reinstatement

If a contribution in default beyond the grace period and the Certificate has not attained cash value, the Certificate is said to have lapsed.

Certificate may be reinstated within six (6) months up to five (5) years from the Lapse date depending on the plan. Note: Subject to Underwriting assessment.

Reinstatement fee is charged RM 50.00 per transaction (subject to plan) 

SPM Allowance

For Takafulink Education plan, participant is eligible to claim the SPM allowance for the SPM exam result. The Certificate effective date should be prior to the exam date and the certificate should be inforced until current date.

Participant may give the consent to company to process personal data for marketing purposes.
